What is A Routine Follow Up Visit?
The Routine Follow-Up Visits are for returning patients with stable conditions who require ongoing care from primary care providers. Unlike the Evaluation and Management and Physical Exam services, during a follow-up visit, we assess the effectiveness of your treatment plan and address any ongoing health concerns you have.
How Does Routine Follow Up Visit Work?
In our Routine Follow Up Visits service, we:
Monitor Your Symptoms
We review preexisting symptoms discovered during a physical exam and test for new ones that could be hindering your recovery.
Establish Continuity Of Care
By scheduling a follow-up visit, we incorporate any new information from your most recent visit into your ongoing care plan.
Answer Questions And Address Concerns
During these visits, we address patients’ questions and concerns, and develop solutions you can easily implement to drive your care forward.
Better Medication Plan
In a routine follow-up visit, we’ll review all the medication you were prescribed and accurately integrate them into your treatment plan.
